application provides a completed form 8850
that is electronically signed without need to print, sign or mail
form 8850.

Start Guide
On the login page for “MCM’s W.O.T.C. Online:
Login using your User Name and Password.
Click “Accept” on the “Electronic Records and Electronic
Signature Agreement”
The
employee needs to have the option to not accept the agreement.
If the
employee does not accept then have them complete a manual form
8850 and the MCM Pre-screening Worksheet.
Employee Enters
Information
The next 2 pages are the same as form 8850 and
the MCM Pre-screening Worksheet.
3. Employee enters their personal
information including DOB and SSN
4. Click “Next”.
5. Employee answers the WOTC
Pre-screening Questions
This
page asks for benefit dates and additional references
Simply
ask the employee to complete as much as they can remember.
6. Click “Next”.
Electronic
Signature Homepage in 3 Steps
There are 3 steps on the “Electronic Signature
Homepage”
7. Step 1- Click “View form 8850”;
View
the form populated with the employee’s personal information.
Then
click the box next to “I have read and understand form
8850”.
8. Step 2- Click “E-sign” to apply your electronic signature to
form 8850.
9. Optional: The employee can view their electronically signed
form 8850 page 1.
To do
so click “View Signed 8850”.
Then
click the box next to “I have viewed my E-signed form 8850”.
10. Step 3- Click “Submit” to submit the form.
11. The programs logs you out and are taken back to the login
window.

First Day: On the day the job
is offered, or the work start date, the new hire arrives at the
work location.
Log In:
The Hiring Manager logs into MCM’s W.O.T.C. Online wizard at:
After logging in you arrive at
the Hiring Manager Homepage for MCM’s WOTC Online system. The Hiring
Manager then clicks "Start Here" to get the forms started for a
new hire.
The
Hiring Manager then inputs the first and last names
of the new hire and clicks, "next".
This is a view of
the employee's log-in credentials.
The Hiring Manager can print this page for later use,
or proceed to allow the employee to complete the forms right away.
The Hiring Manager then gives use
of the computer to the new hire
to enable completion of their MCM Online W.O.T.C. form.
The new hired sits down and clicks
"Next".
The new hire is then required to verify their identity by
checking the box, to affirm the statement.
To proceed click "Next".
Once the new hire clicks "Login To Start"
logging in is automatic.
Then the employee clicks to “Accept” the “Electronic
Records and Electronic Signature Agreement”
If the employee does not accept then
please provide them with a paper form 8850 and MCM Pre-screening
Worksheet.
Employee
Information:
Please
provide your basic contact information by completing the “Work
Opportunity Tax Credit Eligibility Worksheet”; including Date of
Birth, Social Security Number, Home Address and Home Phone Number
as a newly hired employee.
The next 2
pages are the same as form 8850 and the MCM Pre-screening
Worksheet.

Input Error-
Social Security Number: Please input all 9 digits of
your Social Security Number. The system will not allow you to
complete the online process without this information. Your
personal information is private, secure and 100% confidential.
Input Error-
DOB: If your data of birth is not between 16 and 100
years of age then a reminder will ask you to return to the input
screen to correct your information. Tip: You may have input the current year fro your
birth year, a common typo in this case. Click the "back" button to
go back and correct your entries.
Input Error- Start Date: Please
input the current date if you are not sure of your exact start
date. Though the system will accept any date 15 days in the
past or 60 days in the future. Please note: WOTC paperwork must be
completed on or before the day the job offer is made. Click the
"back" button to go back and correct your entries.

protect your privacy.
At the end of the survey process click “Continue”.

For

The new

read the release statement authorizing MCM, on behalf of your
employer, to verify your
eligibility tohelp gain W.O.T.C. tax credits for
the company.
